The gift of a healing heart
Proper recovery from a serious heart-related illness is so vital to a patients’ ongoing quality of life. Heart Path is a cardiac rehabilitation program that provides patients with that important next step. But a growing number of patients who need Heart Path are not able to afford it. In 2007 and 2008 alone, Heart Path welcomed a total of 500 new patients. In addition, there are 360 patients in the maintenance program. According to staff estimates, at least 20% of people who need to be in Heart Path require some financial assistance. Even patients on Medicare, who do not have supplemental insurance, need assistance to fill the gap.
Since February is Heart Health Month and March is Pulmonary Rehab Month, we encourage all of you to do things to make your heart and lungs healthy. Just as important, you can give the gift of a healing heart by helping a patient who cannot pay attend Heart Path. About Heart Path
Heart Path provides individually tailored care, allowing patients to take a lead role in their own healthcare, including a detailed look at their crucial numbers (blood pressure, blood glucose, cholesterol and body mass index) and how to reach optimal ones.
|
This includes personal assessments to help develop care plans which can include:
- exercise therapy
- nutritional counseling
- psychosocial counseling addressing issues such as depression, anger, and quality of life
- smoking cessation counseling
- lifestyle behavior modification
|
Heart Path also features monitoring by and/or access to an array of highly trained staff including:
- cardiologists
- pulmonologists
- physical & respiratory therapists
- registered nurses
- dietitians
- psychologists
- exercise specialists & physiologists
